The plan, the records and the visits in one place, with reports built from them.

A care plan in ELSA is the person's record, not a document about them. The daily visits, the incident and assessment records, and the review all sit against the same person, so a weekly or monthly report is drawn from what was actually recorded rather than written from memory.

What this replaces today

  • A care plan in a Word file, a risk assessment in a folder, and visit notes in an app that does not talk to either.
  • An emergency contact typed as a single line with the name, address and phone run together, which nobody can actually ring from.
  • Working out at review time how many calls were missed last month by counting backwards through paper.
  • A carer whose login shows them the whole caseload, because the system has no way to give them only the people they support.

How it works

Build the plan, or start from a template

Eight sections covering identity, needs, risk, daily support, outcomes and review, with six starting templates you then make specific to the person: domiciliary, dementia, learning disability, mental health, end of life and reablement.

Staff record visits and events as they happen

A visit carries when it started and when it ended, which call it was, and how it went: completed, partial, refused, no access, missed or cancelled. Records for incidents, assessments and capacity attach to the same person.

Reports are drawn from those records

The weekly and monthly reports cite the logs they came from, state their own coverage, and refuse to write a period where nothing was recorded.

What you get

27 record types, in adult CQC language

Incidents, accidents, falls, body maps, safeguarding concerns, risk assessments, moving and handling, nutrition screening, skin integrity, continence, mental capacity assessments, best interests decisions, medication errors, advance care plans, hospital passports, spot checks, complaints, compliments. Adult records never mention placements or young people. That is supported living vocabulary, and mixing the two is how a form stops making sense.

Missed and refused calls are countable, not free text

Commissioners and inspectors ask how many calls you missed. An outcome you can count answers that; a note somebody typed does not. Visit duration is recorded too, because that is what a commissioner is paying for.

Capacity recorded decision by decision

A mental capacity assessment is tied to the specific decision, and each of the four statutory limbs needs its own evidence: understand, retain, weigh, communicate. It cannot be recorded as a blanket judgement about a person, because under the Act it never is one.

Contacts you can actually use

Name, relationship, phone and address as separate fields for emergency and main contacts, plus a list of healthcare professionals. Where an old record ran them together the split is offered, never guessed: a blank beats an invented address.

Care plan notes that propose rather than change

Record a review by microphone, from a Teams meeting's own transcript, or by typing it up. ELSA compares it with the current plan and proposes specific edits, each carrying the quote that justifies it. Nothing reaches the plan without approval.

Carers see the people they support

A carer is limited to those they are assigned to, named reviewer for, or listed as a contributor on. That is enforced on the server, not by hiding a filter. A plan they may not open is indistinguishable from one that does not exist.

What stops it going wrong

A period with no records is refused

No visits logged means no report, not a plausible one. The model is never called.

Coverage is part of the report

It says three of seven days had no visit logged, and that one call was recorded as missed, rather than writing smoothly past both.

Proposed changes stay proposals

Everything ELSA suggests from a review meeting is pending until a person approves it, and the approval is recorded against them.

Attachments are never searchable

A document attached to somebody's care record is never indexed and can never be retrieved into a tender answer or a chat reply. That boundary is deliberate and absolute.

Who it is for

Every paid plan: capability is not gated, volume is

For domiciliary, residential, supported living and complex care providers regulated by CQC.

Records and visits are deliberately on every plan, because they are the day job. What the plan sets is how many people you support: 25 on Pro, 50 on Growth, 100 on Group. Care plan notes are included from Pro.

This is not a medication administration chart. MAR is a different surface and ELSA does not pretend to be one.
